Careplus

CarePlus is a programme for patients with chronic (ongoing) conditions. Patients on CarePlus are eligible for at least four subsidised visits to their general practice. If you are a CarePlus patient you will have an initial assessment, where you will talk to your Doctor or Nurse about your health. Together you will develop a plan for your care. This may involve setting goals to make changes to your lifestyle or other things you can do to keep you as well as possible and to minimise any complications with your condition. Regular follow-ups will be included in the plan.

If you think you may be eligible for CarePlus ask your doctor or nurse about it. If you are not eligible it may be that they can help you in other ways or review your existing care. CarePlus services are provided at a reduced cost from usual consultation costs.

High User Health Cards


If you have a high user health card you may be able to move onto CarePlus when your card expires.
